Registation will close on Wednesday 10 June, 7pm AEST. 

Please read all of the information below before submitting your registration. Once you have read the info, please click on the register now button below.

Registration is only available via the online registration form. When you submit your registration you will receive an acknowledgement email advising you of your registration reference number after successful submission. If you do not receive the acknowledgement email, it may be due to an error in your email address. Please contact the Congress Organisers if you have any queries.

*Please note - only current financial CNSA Members are eligible for the Winter Congress Discounted Member Registration Rate. If you are currently a member from the 2014 membership year and intending to rejoin when the new membership year opens 1 April 2015 - 31 March 2016 you are eligible for the Discounted Member Registration Rate. If you have become a new, first time member since 1 October 2014 you are eligible (your membership automatically goes to 31 March 2016) for the Discounted Member Rate.
If you have not renewed in 2014 go to www.cnsa.org.au, enter your username and password to login and renew.
You must be a financial member of CNSA at the time of Congress to access the Discounted Member Rate.

 **Become a New Member of CNSA (only available to those who have not been a previous member) at the Discounted Member Fee of $90, that is a 50% saving. ***A person is eligible for the student rate if they are undertaking full-time undergraduate or post-graduate studies with a stream of cancer care. Documented evidence of full time status (e.g student card or letter 
from University) is required upon registration and is subject to approval. (Automatically includes a student membership to CNSA)

Workshop Registration includes

• morning tea, Lunch, and afternoon tea
• name badge and attendance certificate (sent electronically).
• Any workshop materials provided by the facilitator 

PAYMENT

Registrations can be paid by credit card (Visa and MasterCard are accepted). A 2.5% credit card surcharge applies to all credit card payments. You can also arrange payment via electronic funds transfer. No surcharge applies to pay via EFT but payment must be received within 14 days of your registration date. All prices quoted are in Australian dollars and include GST.

CANCELLATION

Cancellations received in writing to the Congress Organisers’ 45 days prior to the Congress commencement date will receive a refund less a processing fee of $120 including GST. No refund will be given after this date; however, a delegate substitution may be permitted. All cancellations and substitutions must be made in writing to the Congress Organisers.

LIABILITY

In the event of industrial disruptions or natural disasters, the Cancer Nurses Society of Australia and Chilli Fox Events accept no responsibility for any financial or other losses incurred by the delegate.

DISCLAIMER

The Congress program and registration information is correct at the time of publication. However, the Organisers reserve the right to change the information where necessary without notice.

PHOTOGRAPHY AND RECORDING DISCLAIMER

During your attendance at the CNSA 2015 Winter Congress, your image or voice may be recorded via video, photography or by any other means (“recorded”) by an official representative of the CNSA, which may be distributed or published at the discretion of the CNSA. If you do not wish to be recorded, you are required to formally advise the CNSA by emailing cnsa@chillifoxevents.com.au

INSURANCE

Registration fees do not include insurance of any kind. Delegates are strongly advised to take out their own travel and medical insurance. The Organisers will not take responsibility for any participant failing to insure.
